Planning Your First Visit
When you are planning your first visit with one of our specialists you will no doubt have many questions and concerns. It is a good thing to make a list of everything you would like to talk about so we will be able to answer all your questions. Feel free to bring any and all family members you will feel comfortable with.
When you arrive, you will be asked to provide us with your historical personal and family medical information by filling out a form. This is for your doctor's use and will remain with your medical records.
You will need to give us your billing information such as your address, date of birth, etc. We will want to copy any insurance cards so bring them along, as it is very important to get the correct information. If your insurance requires a referral form from your primary care doctor, we must have that before our specialist can examine you.
If you have been asked to bring x-rays, reports and records from other locations, be certain you have those with you since they will be relevant to your visit with us.
